HealthDay.si is an annual event dedicated to encouraging digital innovations as a way of improving the healthcare system. It is hosted by our partner community of medtech, biotech and health IT innovators from Slovenia, taking place today, 10th of October at the Medical Chamber of Slovenia in Ljubljana.
We have been invited as the guest speakers to share the distinctive value of our Vitaly Managed Care solution through the eyes of a healthcare profesional and supported with a recent customer case study of digitisting 5-year recovery programme of cancer patients in the UK.
Evidence-based outcomes of digitally supported care coordination
Our medical consultant Julij Šelb opened the presentation with demonstrating the value of digitally supported solutions for coordinated care through evidence-based studies, showing the positive health outcomes when treating and managing outpatient care with various diseases and conditions (advanced cancer, diabetes, hypertension and more).
Building a collaborative platform for the residents in NW England
Mr. Pete Smith – our UK customer representative (NHS, North West of England) has introduced the ultimate goal of WelLPRES project, which lies in building a platform for all teams (clinical and non-clinical) and all residents of Lancashire and South Cumbria to efficiently collaborate on improving health and social care.
The aim of our first joint project within WelLPRES is to digitise the analogue approach of 5-year cancer recovery programmes through a holistically designed solution that allows personalised and coordinated care of the patients, recovering from breast, prostate and colorectal cancer.
Tailoring the application to the needs of NHS professionals
The overall high engagement of the care professionals in the workshops of giving insights how the WelLPRES system works was crucial to shape the digital solutions with them and for them.
The professional view within WelLPRES is accessible through existing hospital Trust systems (i.e. no additional log in). It supports a care planning approach, built around Vitaly Managed Care caremap functionality. This supports professionals in having a standardised approach to core care delivery through the digital platform, allowing personalisation based on clinical judgement and patient-reported needs.
Using Vitaly Managed Care, the professionals are able to access clinical information, test results and reports pulled into the system from LPRES. This means that the clinical data here is a cumulative and system-wide clinical report, pulling information from the patient’s GP, hospital clinical team, and in the future other care providers including social care and, where the data is available, the voluntary, community, faith and social enterprise sector.
Following care pathways already being planned
In conclusion, Mr. Smith mentioned the project will be continuing with further cancer pathways in development, and they are already in the process of scoping heart failure, mental health and collaborative paediatric care for development during upcoming years.
