PATHFINDER – Proactive Crisis Management in Health & Care

PATHFINDER is an innovative crisis management system designed to enhance the resilience of health and care organizations. This project focuses on creating a proactive framework for managing crises through advanced technology, multidisciplinary teamwork, and continuous stakeholder engagement. By providing comprehensive crisis management tools, PATHFINDER enables organizations to plan, prevent, manage, and recover from crises more efficiently, improving both public health outcomes and organizational resilience.

 

 

Problem Being Solved

Healthcare systems face significant challenges in responding to crises, from natural disasters to pandemics. Many current solutions are reactive, which can lead to delayed responses, inefficient resource use, and negative outcomes. PATHFINDER addresses these gaps by offering a proactive, technology-driven approach to crisis management that can adapt to changing circumstances in real time, improving both operational efficiency and patient care outcomes.

 

Solution and Technology

PATHFINDER integrates a comprehensive suite of tools designed to streamline crisis management:

  • Control Center: A decision support system that provides real-time data analytics, business intelligence, and workflow management to senior representatives, staff, and stakeholders.
  • Citizen’s App: Offers a central point of access for citizens to receive public health updates, care pathway information, and engage with an AI-powered chatbot for personalized support.
  • Advanced AI and Machine Learning: Optimizes decision-making by providing predictive analytics, automating workflows, and adjusting care pathways in real time based on available data.
  • Multidisciplinary Collaboration Tools: Encourages teamwork and continuous stakeholder involvement for improved crisis response and coordination.

 PATHFINDER is part of the DYNAMO PCP project, which has received funding from the European Union’s Horizon Europe Research and Innovation Programme under the Grant Agreement No. 101095516.
