What happens with the cancer survivors after finishing their in-hospital treatment? How can they manage their condition with confidence and stay in touch with their care teams when they really need them? How can an integrated approach between primary and secondary care help in minimising unnecessary outpatient appointments and support more sustainable systems? 

Dr Colin Brown, CCIO at University Hospitals of Morecabe Bay NHS Foundation Trust, presents an example of how several NHS Trusts in the Lancashire and South Cumbria (NW England) tackle these challenges to achieve safer and more accessible services for breast, bowel and prostate cancer survivors. The webinar was recorded on 17th June 2021 at the Dutch ICT&health event in the format of a best practice from a foreign country.
